Registration for Amateur Radio License Testing

We collect this information in order to prepare the LARC-VEC Form 605 and other testing paperwork so please fill in all blanks. Preparing the forms in advance greatly speeds the testing session but we gladly accept walk-in candidates. Our testing facilities are provided by Wickline United Methodist Church and our use of them could be canceled on short notice – we may use this information to contact you. Note: If you wish to conduct business with the FCC, you must first obtain an FCC Registration Number (FRN) through the FCC's COmmission REgistration System (CORES). This number will be used to uniquely identify you in all transactions with the FCC. Current licensees may have their FRN printed on their license if they have a PDF or printed copy; if not, FRNs can readily be determined by visiting an online callsign server. If you have a GMRS or other FCC license, you already have an FRN - check your license documents to determine the number!
Have you ever been convicted of a felony in any state or federal court? If you answer "Yes", then you will have to submit an explanation to the FCC before they will process your application. (See provided instruction sheet.) You must answer this question if you are applying for a new license, applying to upgrade your existing license, or requesting a call sign change. You do not have to answer this question if all you are doing is renewing your license or changing your name or mailing address.
